The Clinic for Internal Medicine has 220 employees including specialists in internal medicine- 43; doctors on specialization- 16; secondary doctors- 5; nurses/technicians with a high health school- 27; nurses/technicians with a higher nursing school- 2; nurses/technicians with nursing school – 123; non- medical workers-5.

 

CONTINUITY IN KIDNEY TRANSPLANT PROGRAM

Living- donor kidney transplant program has been continuously carried out in the University Clinical Center Tuzla since it was started on September 15, 1996. Up to date 225 transplant surgeries have been successfully performed. A multidisciplinary team consisting of specialists in nephrology, urology, anesthesiology and related diagnostic branches are actively involved in the kidney transplant program.There are many certified experts in the field of transplantation medicine among them, and some of them have European transplant certification.

Once met all the necessary requirements and the criteria of Eurotransplant a cadaver transplant program that included experts from Clinic for Internal Medicine started in 2006. So far 35 cadaver kidney transplants have been performed at the University Clinical Center Tuzla. Grafts and patients survival rates in kidney transplant recipients is 74% after five years.

Several master’s theses and two doctoral theses were defended on the theme of kidney transplant.

The center for dyalisis is a provider of dialysis and related health services to all chronic and acute dialysis- dependent patients with kidney failure. Al methods of extracorporeal circulation are offered in order to treat acute and chronc kidney failure, acute exogenous intoxications and other severe systemic diseases and mutiorgan failures. Since 2002 dialysis center has been appointed reference Fresenius MC for the Federation of Bosnia and Herzegovina. There are 138 patients on hemodialysis in the University Clinical Center Tuzla.

ScopeGuide- assisted colonoscopy

ScopeGuide assisted colonoscopy is a diagnostic as well as a therapeutic procedure performed to evaluate the large intestine including entering the final part of the small intestine. The colonoscopy device helps the doctor to determine the exact position of the device in the abdominal cavity of the patient, therefore to complete the procedure as fast as possible. The procedure enables efficient performance while avoiding entering the so- called arches, which make the procedure difficult and painful for the patient.

Capsule endoscopy

Capsule endoscopy helps the doctor to see the small intestine pathology completely painless and with a high accuracy of diagnosing even the smallest changes in the small intestine.

Intragastric balloon placement in the treatment of pathological obesity

The balloon is placed endoscopically in the stomach and then filled with saline and where allowed colored with methylene blue dye. The balloon is filled depending on the patient’s body weight. The balloon is being removed from the stomach six months later. This method is not suitable for patients whose obesity is related to genetic and hormonal disorders.

FibroScan- Transient elastography

Transient elastography or FibroScan is a non- invasive ultrasound technology that measures the liver tissue damage in various diseases, and assess the amount of liver fat (steatosis grade). This procedure in a non- invasive method allows the doctor to determine the degree of fibrosis, and to diagnose cirrhosis of the liver. It is not used for elastography of focal liver changes such as tumors.

Etanol ablation of changes in the liver

Etanol ablation procedure is primarily used for a non- invasive treatment of primary tumors of the liver- hepatocellular carcinoma, and to a lesser extent metastases in selected cases. The procedure can treat one change of a maximum diameter up to 50mm, or 3 smaller changes with total diameter of 60 mm.

Stress echocardiography of the heart

Stress echocardiography is an established technique for assessing coronary artery disease.

The Clinic for Internal Medicine is the place of science and education as well as a teaching base for Nursing School and Medical faculty. The health care professionals from the Clinic for Internal medicine are active members of various scientific and research societies.

CENTER FOR INFORMATION- TRANSPLANTATION

This Center presents the development of transplantation medicine at the University Clinical Center Tuzla. Health professionals engaged in the center have actively worked to create a unique waiting list and a database for all patients who went through the transplantation surgery. They role is also the coordination with medical centers in the region, colleagues and patients. Information Center which has been operating since October 2008, was created to consolidate data from the field of the transplantation medicine, as well as to establish contacts and network of transplantation medicine centers. One of the main tasks of Center is to collect data from all segments of transplantation medicine, and make them available to the patients and health professionals dealing with transplantation medicine. The Center is also the place where transplant medical education and training is conducted.

Outpatient clinic of the Clinic for Internal Medicine with capacity of 6 beds provides complete health care services in the field of diagnostics. It is open to provide costs and time- effective diagnostics and a 24- hour observation of patients without their long staying and hospitalization.

Every day 5 doctors and nurses/technicians are engaged in the outpatient clinic workflow which includes fast diagnostics, specialist examinations, ultrasound, and consultations with doctors of different specialties and sub- specialties.

Intensive care unit with 14 beds offers comprehensive treatment, hemodynamic monitoring, ultrasound diagnostics and continuous follow- up of patients. Ultrasound of abdominal and urinary tract, transthoracic echocardiography, mechanical support, pace maker simulation, electrocardioversion, cardiopulmonary resuscitation and parenteral/ enteral nutrition of patients in critical condition.

Health professionals at the unit deal with rational diagnosis and treatment of the most serious disases in internal medicine, including acute and life- threatening conditions that require critical intensive care and therapy.

On average, 1.500-1.800 patients are treated annually in the Intensive Care Unit with a coronary unit, with an average hospital stay of 3.0-3.5 days.

The department has 39 patient beds with a semi-intensive care unit and a unit for interventional ultrasound. The Department of Gastroenterology provides the most modern diagnostic procedures in the field of upper and lower gastrointestinal endoscopy and interventional ultrasound. From the wide range of procedures, we highlight interventional endoscopy, endoscopic retrograde cholangiopancreatography (ERCP), endoscopic ultrasound, video capsule endoscopy and interventional ultrasound with drainage procedures. . Supported by powerful radiological, laboratory and immunological diagnostics, the Department of Gastroenterology is today able to successfully diagnose and treat most diseases of the digestive tract.

Electrocardiographic diagnostics (standard ECG, averaged ECG, 24-hour ECG, stress ECG), vectorcardiogram, ultrasound of the heart, catheterization of the right and left ventricles, transesophageal ultrasound of the heart in preparation of heart defects for surgery. The cardiology department has 60 patient beds. Health professionals at the department deal with non-invasive diagnostics and treatment of cardiac diseases using ergo test, holter and heart monitor. The invasive methods includes transesophageal ultrasound of the heart, which better visualizes the heart structures that is important for cardiac surgery, as well as for the postoperative evaluation. There are 7 specialists in internal medicine, two professors of internal medicine, 3 masters working in the department, of whom two have the title of European cardiologists and one has the title of fellow. All specialists are active members of the Association of Cardiologists of Bosnia and Herzegovina, as well as the Association of Cardiologists of Europe and the World. Until now, the professionals from the department has participated in numerous research projects, the results of which have been presented in reference journals. In cooperation with the Faculty of Medicine of the University of Tuzla, in 2003, the first textbook “Cardiology” was published in 2003. The Department of Cardiology organized Congress of cardiologists of BiH with international participation, two international symposiums and two professional meetings of cardiologists of BiH. In its educational program, the Department of Cardiology organizes the Ultrasound Heart School for internal medicine specialists in BiH, as well as for its own staff. This is the only ultrasound heart school in Bosnia and Herzegovina that has been working continuously since 2002.

Health professionals from the department treat patients suffering from different kidney diseases and failures by using dialysis methods.

The department has 27 patient beds.

The dialysis center was founded in 1970 in Tuzla as the third open dialysis center in the former Yugoslavia. Since then, the center has been working continuously and caring for all chronic and acute dialysis- dependent kidney patients. All methods of extracorporal circulation are performed in order to treat acute and chronic renal failure, acute exogenous poisoning and other sever systemic diseases and multiorgan failures: standard low flow and high- flow hemodialysis, online hemodiafiltration, intermittent, slow low- efficiency dialysis and continuous dialysis, intermittent, continuous and cyclic peritoneal dialysis, therapeutic membrane plasmaphresis, the isolated ultrafiltration, introduction of temporary and permanent vascular catheters and management of the kidney transplant waiting list.

Following diagnostic procedures are presently available at the Department:

  • Abdominal ultrasonographic evaluation of the urinary tract,

  • Doppler sonography of renal artery,

  • Ultrasound- guided kidney biopsy,

  • Ultrasound- guided puncture and evacuation of kidney cysts

Th Department of Nephrology and Hemodyalisis has ten doctors specialists, four university professors, one doctor with PhD, four doctors with masters of medical science, and one doctor specialist in internal medicine.

There are 37 highly- trained dialysis nurses/technicians, two dialysis equipment service technicians and one administrative person actively involved in the department workflow.

The Department provides continuing medical education activities important for the medical profession of nephrology.

Health professionals from the department deal with the treatment of inflammatory and degenerative diseases of the musculosceletal system (rheumatoid arthritis, psoriatic arthritis, spondylarthritis, systmic lupus erythematosus and systmatic diseases of the connective tissue, severe forms of osteoporosis), ultrasound diagnostics of the locomotor apparatus, ultrasound- guided puncture for intra- articular, extra- articular and canalicular infiltrations of joints.

A total of 24 patient beds are available in the department of rheumatology.

A highly professional staff of the department offers the treatment, monitoring and clinical diagnostics (laboratory, immunological, microbiological, X-ray) for patients suffering from rheumatic disorders. Patients with systemic connective tissue diseases receive pulse- doses of cyclophosphamide, as well as cyclic pamidronate therapy in the treatment of severe osteoporosis with threatening fractures in the outpatient clinic.

Professional staff regularly provide patient education how to cope with disease-related problems of everyday life, with stress and with suffering.

The department is a teaching base for residents, doctors on internship, students of the Faculty of Medicine, Nursing college and Nursing school.
